I've tried field stripping the gun, taking the barrel off, bolt out and trigger assembly off and reassembling the gun.

The dishing happens when the chamber is rough. Get a 12-gauge brush and wrap some fine steel wool around it and chuck it in a power drill. Run it in the chamber for a minute.
